In-Home Care Services for Seniors
San Jose Caregivers/ Care Managers
Understandably, most seniors prefer to stay in the comfort zone of their own homes, where they are comfortably situated and safe. This is more doable than you may think. We can arrange for medical and non-medical visits from registered nurses, physical therapists, social workers, and home care aides. We personally monitor daily our caregivers, to the extent of actually supporting them to ensure optimal care to all our patients.

Same Day Assessment and Staffing:
Because you care about getting yourself or your loved one a help right away, we assigned our Program Manager and Director to give you an in-home free consultation the very same day you call. It will only take a few minutes for the visit and at the end of the consultation. If you decide to move forward, your care manager and coordinator will make a comprehensive assessment in order to give you the specific service you need at the lowest cost and on your specific budget.
Your SCC At Home, LLC Director and Coordinators can start providing services right away after we were able to conclude and meet your needs for finality.
